All players and/or officials registering or nominating for an event must be registered & financial with a BHA affiliated Club and registered & financial with the Brisbane Hockey Association and with Hockey QLD through revSport.
# I understand that Exemption Requests can be submitted 2 days prior to respective trials commencement and 7 days before expected round game.
# I understand that applications for Exemption from BHA Representative trials will not be accepted for consideration if ALL relevant information is not completed.
# I acknowledge that Applications for Exemptions submitted after the closing date may not be accepted. Supporting evidence should be included with the submission, and additional information may be requested.
# All information provided in this Application for Exemption is true and accurate and I understand that approval for Exemption is at the discretion of BHA Director of Representative Hockey.
