INCIDENT REPORTING

All incidents relating to child safety, member protection, complaints, misconduct, discrimination, harassment, bullying, or breaches of BHA policies should be reported to the individual's club as soon as practicable after the incident occurs.

Steps to Submit a Report

1. Record the details Include:

  • Date, time and location of the incident
  • Names of those involved and any witnesses
  • A factual description of what occurred
  • Any actions taken at the time

2. Submit the report to your club Provide the report and any supporting information to the appropriate club official.

3. Club review and submission to BHA The club will review the information and submit the report to the appropriate BHA Safeguarding & Integrity Officer for assessment and management in accordance with BHA policies and procedures.

4. Provide further information if requested The club or BHA may request additional information to assist with the assessment of the matter.

Reports should be made as soon as practicable after the incident occurs.
