- F13.1 Application for Registation of Club Teams - right-click for OOo, MSWord or PDF format
- used to apply for teams in specific divisions for 2008
- use the OOo or MSWord version to reply via email
- use the PDF version to fill in and fax or post
- F13.2 Team Registration Form - right-click for OOo (21kb), MSWord (85kb) or PDF (89kb) format
- use this form to register players in specific teams at the beginning of the season (2008 version)
- F13.3 Player Registration/Re/De-registration Form - right-click for OOo, MSWord or PDF format
- use this form to register new players after the season commences; and
- use this form to re-register a player from one division to another; and
- use this form to de-register a player, i.e. the player will no longer play with your Club in the current season
- F13.4 BHA Member Registration Form - right-click for OOo, MSWord or PDF format
- Club Officials Form Right-click for OOo, MSWord or PDF format
- a completed form must be submitted by the Club Secretary each year and anytime the persons holding these positions changes
- use the OOo or MSWord version to reply via email

Notes:
- OOo - refers to OpenOffice.org v2, the free Open Source MSOffice compatible, multi-platform Office Suite for MSWindows, Linux and other Operating Systems. The format used by OOo is an international standard and file sizes are generally smaller than MSWord.
- MSWord - refers to the Microsoft Word 97/2000/XP format. This format can also be opened using OpenOffice.org, Ability Office, etc.
- PDF - The viewing & printing of PDF documents requires that a suitable "Reader" application is available on your system. The free Adobe Reader can be downloaded from the Adobe Website. A much smaller and faster Reader for MSWindows systems is FoxIt Reader. FoxIt Reader is also free and can even be run with or without installation.
