BHA Forms - 2007 Season

• F13.1 Application for Registation of Club Teams
  Right-click for OOo, MSWord or PDF format.
  - used to apply for teams in specific divisions for 2007
  - use the OOo or MSWord version to reply via email
   
• Club Officials Form

  - a completed form must be submitted by the Club Secretary each year and anytime the persons holding these positions changes. Use the OOo or MSWord version to reply via email.
   
• Proxy Form Right-click for PDF format.
  - used to appoint a proxy for meetings.

• Team Fees 2006 - Instalment Form (Tax Invoice)

  - A completed copy of this form is to be submitted with fee payments - three (3) instalments.

  3. De-registering a player, i.e. player no longer playing for club.

  BHA General Committee meeting of 23rd Jan 06.
   
• BHA Competition Rules & Requirements - so far these have not altered since the 2005 version. A 2006 version will be published following the BHA Veterans Committtee meeting as there may yet be some alterations specific to the Veterans competition.

While most of these have now been filled, refer to About - BHA Office Bearers, applications in writing may be considered for those that have not. However it might be wise to check with the BHA General Secretary for the latest details before doing so.

Hockey Queensland Forms

These forms appear on the BHA Website for the convenience of the members of BHA with the permission of HQ.  When submitting either one of these forms to BHA the form must be accompanied by a completed F13.4 BHA Member Registration Form (refer elsewhere this page).

• Dual Registration Form 2006 (PDF)
• Alternative Association Registration Form 2006 (PDF)

Dual Registration Forms are only required when a player actually participates in the regular competitions of more than one Association. Alternative Association Registration Forms are only required when a player wishes to play for a Rep team of an Association other than the one in which he is registered and participating. These forms can also be downloaded from the HQ Website.
