While most of these have now been filled, refer to About - BHA Office Bearers, applications in writing may be considered for those that have not. However it might be wise to check with the BHA General Secretary for the latest details before doing so.

Hockey Queensland Forms

These forms appear on the BHA Website for the convenience of the members of BHA with the permission of HQ. When submitting either one of these forms to BHA the form must be accompanied by a completed F13.4 BHA Member Registration Form (refer elsewhere this page).
Dual Registration Forms are only required when a player actually participates in the regular competitions of more than one Association. Alternative Association Registration Forms are only required when a player wishes to play for a Rep team of an Association other than the one in which he is registered and participating. These forms can also be downloaded from the HQ Website.

Notes:
OOo - refers to OpenOffice.org v2, the free Open Source MSOffice compatible, multi-platform Office Suite for MSWindows, Linux and other Operating Systems.
MSWord - refers to the Microsoft Word 97/2000/XP format. This format can also be opened using OpenOffice.org, Ability Office, etc.
PDF - The viewing & printing of PDF documents requires that a suitable "Reader" application is available on your system. The free Adobe Reader can be downloaded from the Adobe Website. A much smaller and faster Reader for MSWindows systems is FoxIt Reader. FoxIt Reader is also free and can even be run either with or without installation.
