F13.1 Application for Registration of Club Teams - This form is used to register a club's teams each season. The information gathered from these forms is used to allocate teams to divisions therefore allowing for the season's draw to be produced. View the 2005 Allocations to date.
F13.2 Team Registration Form - This is a compulsory form that is used to register players in their respective teams and must be submitted prior to the first fixture in each division/competition. Right-click this link to download in MS Word format.

CCYP&CG - 'Blue Card' Commission for Children & Young People & Child Guardians - All persons over 18 who are involved with children under the age of 18 must obtain a suitablity notice. This includes club and association officials, coaches and managers. To find out more and/or to download forms click the above link.
Member Registration Form - It is desirable that all players complete and sign one of these forms each season. It is a mandatory requirement however that players who wish to be considered for HQ Dual Registration have completed, signed and submitted one of these BHA Forms via their club.
